Content Reuse Terms and Conditions

By requesting permission to reuse the CWH Multi-element Intervention graphic from the Window into CWH: An Awareness Tool you are acknowledging and agreeing to the following:

  • The graphical representation of the Cognitive Work Hardening treatment intervention remains a copyrighted image owned by Adeena Wisenthal (2020), with all associated rights reserved.
  • The permission request herein is for a one-time use of the copyrighted image for the expressed purpose as outlined in the request form. Any subsequent or different usage requires a separate approval request.
  • Use of the graphic image in either print or electronic form must include a full reference acknowledging the source. Note the JPG image you will receive will include the proper citation to be used.
  • The electronic image may not be shared with a third party beyond the need for the proposed use as stated in the Permission Request Form being submitted.
